We are looking for a motivated and compassionate Practice Nurse to join our large, friendly and supportive nursing team as we continue to expand to meet the growing needs of our patient population.
Our established nursing workforce comprises Advanced Nurse Practitioners (ANPs), Specialist Nurses, Prescribing Nurse Practitioners, Practice Nurses, Nursing Associates and Health Care Assistants, working collaboratively to deliver high-quality patient care. - As a Practice Nurse, you will provide high-quality nursing care, health assessments and treatments to patients, working closely with GPs and other healthcare professionals. You will play an important role in patient education, chronic disease management and health promotion, while working within the NMC Code of Conduct and the Partnership's policies and procedures
Conduct patient assessments, including history taking, physical examinations and vital signs monitoring appropriate to your role.
Deliver a range of treatments and procedures including wound care, ECGs, Doppler assessments, blood pressure monitoring and injections.
Provide immunisations, including childhood and travel vaccinations, in line with national guidance.
Undertake cervical screening and contribute to women's health services.
Provide contraceptive advice, reviews and ongoing care within your competence.
Support the management of patients with long-term conditions such as diabetes, asthma, COPD and cardiovascular disease.
Monitor and review treatment plans, ensuring care remains evidence-based and responsive to patient needs.
Work collaboratively with GPs, pharmacists, specialist nurses and the wider multidisciplinary team to optimise patient outcomes.
Promote health and wellbeing and support patients with lifestyle changes including smoking cessation, healthy eating and physical activity.
Maintain accurate and timely clinical records using the practice's electronic patient record system.
Contribute to the management of clinical stock, vaccines and medical supplies as required.
Share knowledge, support colleagues and contribute positively to the ongoing development of the nursing team.
Maintain a Personal Development Plan and engage in ongoing learning to support NMC revalidation and professional development.
Registered Nurse with current NMC registration.
Full driving licence and access to a vehicle.
Commitment to delivering high-quality, patient-centred care.
Willingness to engage in ongoing learning and professional development.
Experience working within General Practice or Primary Care.
Experience of delivering immunisations and vaccinations.
Experience of cervical screening and contraception care.
Knowledge and experience of chronic disease management.
Nurses from other settings with relevant transferable skills and a desire to develop a career in Primary Care are encouraged to apply.
Please be aware that all employees of the Partnership will be expected to have DBS Clearance.
Requirement for this position is to regularly travel to other sites, therefore you must have your own mode of transport (car).
